Little Scholars Academy Contract
We have 3 payment plans, please select one:*
- Plan 1: Tuition will be paid once a month in full on the 1st
- Plan 2: Tuition will be paid once a month in full on the 15th
- Plan 3: Tuition will be divided into two payments; half will be due on the 1st of the month and the remaining half will be due on the 15th of the month.
If one of these payment plans doesn't work for your situation, please speak to the office manager about making other payment arrangements and we will work with you if possible. If the 1st or 15th happen to fall on a weekend, you will be required to pay on the following Monday. The three-day grace will start on that date. ICCP participants must recertify on time. It is the participant's responsibility to obtain the proper paperwork, have it filled out by one of the directors/assistants, and turn it into ICCP accordingly. If you fail to do so, you are responsible for your tuition in full, until it is reinstated. ICCP participants MUST pay their copay during the month of service, failure to do so will result in termination of your childcare and 20% of the total balance will be added to your bill. The reason for this being this is the fine ICCP will charge LSA for Co-pays collected late or not at all. All account balances must be paid in full during the month they are charged, zeroing your account out by the end of the month. If you carry a past due balance of $500 or higher, your childcare will be terminated and you will have 20 days to pay the remaining balance before an unpaid account fee will be added and turned over to collections. In the event that the account becomes delinquent and payment is not made on accounts owing under the terms of this agreement, and the balance is placed with a licensed collection agency and all parties on contract agree to pay the fees of the collection agency which amount is therefore agreed to be 50% of the outstanding balance at the time the account is placed for collections. The 50% collection agency fee will be calculated and added at the time the account is placed into collections by the client. After attending our childcare facility for six months, your family is given 1 week (5 consecutive days) vacation. Once you have been in attendance for a year, you are given 2 weeks (10 consecutive days or 2 separate periods of 5 consecutive days) vacation. Vacation is to date of enrollment, if you did not use your 2-week vacation in your year, it will not roll over. If vacation has been used, it will begin again the day after the anniversary date. As well as, in order for you to be eligible for your vacation YOU MUST be on time with your payments and not in default for more than 15 days. You are required to give our staff a 2-week notice when using your vacation time, 30-day notice if on ICCP. Your vacation rate is determined by your monthly tuition, divided into the number of days for the month in which you vacation. We then take off the cost of the vacation accordingly. To request vacation time a "Parent Vacation Request Form" must be filled out and submitted. Failure to give proper notice may result in your vacation credit being declined. As your child progresses at our facility, your tuition will change accordingly, i.e. age and or attending school. Any questions you have on what these rates will be, and when exactly yours will take effect can be answered by the office manager. Please note that rates are subject to change at any time and changes to the rate prior to child being of that age will apply. There is a $20 Pre-school fund that you are required to pay once every three months (March, June, September, and December). This is for children 1-5 years of age that attend ones through preschool to cover extra sensory, and party supplies. We ask that you pay this by check or cash, separate from your tuition, and cannot be billed to ICCP. All new enrollments are considered probationary for the first 60 days. This allows time to evaluate whether our program is the right fit for your family and whether your child is a good fit for our group care environment. If, during this period, either party determines that the placement is not a good fit, no advance notice is required for withdrawal. If Little Scholars Academy determines that the care arrangement is not in the best interest of the child, other children, or staff, we may terminate care immediately without notice. When possible, we will make every effort to provide advance notice unless circumstances require immediate action for the well-being of our community. If you choose to remove your child from our care after the probationary period, you must provide a minimum two-week written notice, regardless of the reason for leaving. Upon receipt of your notice, you will be given your prorated remaining balance, which must be paid by your final day. Vacation time cannot be applied toward your final two weeks. If you do not give proper notice, you will be responsible for the next 30 days of full tuition.

This information is for our scheduling and a time frame for how we are going to proceed with our day. LSA Medical Emergency Release and Liability Form
I hereby give my permission for Little Scholars Academy or a representative of the staff to give CPR, to call the child(ren)'s physician, to secure necessary medical care (including the administration of anesthesia if surgery is advised}, to call an ambulance to transport the child to the hospital of their choice, and to otherwise act in my behalf to protect my child if I cannot be reached and/or when delay would be dangerous in case of illness or accident all at my own expense. Unless proven full neglect regarding the attending staff. The undersigned(s) being the lawful parent(s) and/or guardian(s) of the below child, hereby consent to the participation by the child in all day care activities conducted by Little Scholars Academy, herein referred to as LSA, and to the participation of the child in all events related to said activities. The undersigned(s) hereby further authorize(s) emergency transportation by either day care personnel or if necessary by ambulance or other emergency vehicle. I hereby give permission for child care program to transport my child, to an emergency relocation site for staff, teachers and children when it is unsafe to remain at the child care facility. I understand that normal safety rules will be followed, as much as possible, but that the highest priority is to relocate to a safe location. I hereby authorize Little Scholars Academy and its staff to transport all school aged children to and from their enrolled school, knowing that they only transport to certain schools and the school transportation is available to is subject to change at any time. Little Scholars Academy acknowledges that all staff that will be driving LSA vehicles are Boise City Childcare Licensed, 1st Aid and CPR Certified, have valid Driver’s Licenses, and all LSA vehicles are currently insured and have an updated Registration. If there is no medical emergency, the day care staff will first use reasonable efforts to contact the parent(s) and/or guardian(s) before administering or authorizing any treatment. The day care is well child-proofed and the children are consistently well supervised; however, accidents do happen. The undersigned(s) assume(s) all risk of injury or harm to the child associated with participation in the day care and agree(s) to release, indemnify, defend and forever discharge LSA and its staff, employees, and agents of and from all liability, claims, demands, damages, costs, expenses, actions and causes of action in respect of death, injury, loss or damage to the child, or by the child, howsoever caused, arising or to arise by reason of or during the child's participation in the day care. The undersigned(s) acknowledges that when the party responsible for dropping off/picking up their child(ren) is on the premises, that party is also responsible for the actions of the child(ren), and that attention needs to be paid to be sure the child(ren) are where they need to be and they are not performing any actions that may result in injury or damage, to themselves, others, or any property.

LSA Admission Agreement
As your child(ren)'s daycare provider, we agree to: • Give your child(ren) careful attention, affectionate care, and stimulating things to do, such as craft time, story time, nap time, and outside play with weather permitting. • To provide your child(ren) a safe and loving environment • Supplement parent provided food if needed with nutritious snacks and beverages. • Keep you informed of your child(ren)'s activities, growth, and any problems. As a parent, I agree to: • Provide a snack and a lunch. If I do not provide a lunch I understand that the childcare will provide a lunch for my child, with the understanding I will be responsible for replacing that lunch with either another one or pay a charge of $1 .5 0 which will be added to my monthly tuition. • Provide an extra set of clothes and any special equipment, such as diapers, formula, etc. to make the child(ren)'s day run smoothly. • Inform the daycare if someone else other than the parents will be picking up the child(ren). • Report any changes of address or phone number of home or work. • Inform the daycare in advance if the chìld(ren) cannot be brought or picked up at the scheduled time by 1 1 a.m. • Inform the daycare two weeks in advance before removing the child(ren) from the daycare if not daycare tuition will still be held liable for next 3 0 days. • Provide payment by no later than selected contract due date according to the childcare cost sheet st th whether the child(ren) attends or not. Payments are due on the 1 and/or the 1 5 of each month, unless prior arrangements are made. There is a 3 day grace period if payment is late, after 3 days there is a $1 5 .0 0 a day late charge that will be added to your bill, and will back date to the original due date. • I understand that there is a $1 .0 0 per minute per child late charge after 6 p .m. and early charge before 7 a.m. and on the 3 rd offense this will be increased to $5 .0 0 per minute per child charge from there on. Payable with my next monthly tuition. Regular tardiness is subject to childcare termination. And we ask that the child(ren) does not bring gum, hard candies and money to daycare. Thank you!! SUNSCREEN PERMISSION FORM
Parents, We ask that all families provide 1 bottle of sunscreen per child per year to help us keep everyone protected during the summer and sunny months. If your child has a sunscreen allergy or you have a preference on what sunscreen is used due to health reasons you must provide that specific brand to be used ONLY on your child, otherwise we place all sunscreen in a community container and use them on at a time on all children.

Little Scholars Academy Illness Policy- Updated 5/10/23
lt is the intention of this child care program to provide a healthy and safe environment for children. The following illness policy is designed to protect the health of your child, the other children in our care, and the staff. *** All children are required to have a general health check before they may be left in our care including a fever check. Children may not have received any fever reducers prior to drop off to ensure an accurate temperature check at check in. No child in a household will be allowed to be dropped off should any of them present with a fever (100.4 or higher) or are above 99.4 coupled with a persistent cough. *** If a child should become ill while in child care, the child's parent/ guardian will be notified immediately, the ill child and all children from the same household must be picked up from our care within 1 hour of notification. *** A child who becomes ill while in our care shall be separated from the other children and be made as comfortable as possible, while awaiting to be picked up. *** The provider shall make the final decision about whether a child is too ill to remain in our care, or be dropped off *** It is important to have back up child care or be able to leave work if your child is too ill to be in our care. *** Children may not attend if they have had two or more incidents of diarrhea or vomiting within a 24 hour period, and may not attend until they are SYMPTOM FREE WITHOUT MEDICATION FOR 1 FULL CALENDAR DAY (midnight to midnight). *** Children may not attend if they have had a fever (OVER 100.4 ORALLY) until they have returned to a normal temperature (BELOW 99) FOR A MINIMUM OF 1 FULL CALENDAR DAY (midnight to midnight) WITHOUT THE USE OF A FEVER REDUCER. *** If A CHILD is sent home from our facility they will not be allowed to return to our care until they meet the above time requirements of being symptom free, a doctor’s note does not override our decision of whether it is in the best interest for a child to return to care. *** It is mandatory that parents notify us if your child or ANY family member falls ill, is diagnosed, or is exposed to ANY AND ALL ILLNESSES/INFESTATIONS within 12-24 hours and BEFORE the child may attend/return to care. DEPENDING ON WHAT THEY HAVE IT MAY REQUIRE A LONGER EXCLUSION TIMELINE, EX: RSV, FLU, AND COVID, PLEASE NOTE THIS LIST IS NOT ALL INCLUSIVE. The failure to do so will be grounds for termination of your childcare.. LSA will notify all parents if such information is given to us, so that proper prevention precautions can be taken. *** Idaho code 39-1118 requires that LSA have on hand a record of immunization, or reason of exemption within 14 days of enrollment. These records must be updated when additional immunizations are received. *** If at any time there is an increase in illness going around the time without symptoms may be updated on a case by case basis, we understand that this may mean a time period of non-attendance and we apologize for this, but it is in the best interest of everyone to keep contagious ill children out of the building. *** In the case of a positive COVID-19 case within our facility (children, families, or staff) a temporary closure may take immediate effect per Central District Health recommendation; however, all efforts will be made to avoid any type of closure prior to making that decision

LSA Behavioral Guidance Policy
Positive Guidance Philosophy We use positive methods of discipline to encourage self-confidence, self-esteem, and self-control. Examples include: Redirecting a child toward a more appropriate activity. Distracting younger children from potential problems. Recognizing and reinforcing acceptable behavior. Helping children understand that while they are always valued, some actions are not acceptable—and explaining why. Encouraging cooperation and problem-solving through communication. Prohibition of Verbal Abuse No form of emotional or verbal abuse—including shaming, humiliating, or frightening a child—will be tolerated under any circumstances. Prohibition of Physical Punishment No form of physical punishment (e.g., spanking, swatting, hitting, grabbing, shaking, biting, etc.) will ever be used by any adult in the facility. No Punishment Related to Basic Needs Discipline will never be associated with necessary food, rest, or toilet learning. Children will not be punished or shamed in relation to these essential needs. Physical Restraint Policy In accordance with current regulations, physical restraint as a form of punishment is not permitted. We will use de-escalation strategies, supervision, and support techniques to ensure safety. If a child’s behavior presents a serious safety concern, parents will be contacted for immediate pickup. In the case of possible risk to other children in our care physical intervention strategies may be used until an authorized pick up person can arrive to take over care of the child. Time-Out and Isolation Children will never be isolated in closets, locked rooms, or other spaces as punishment. Time-out, when developmentally appropriate, may be used in a supervised area for older children who understand and benefit from a brief break to regain control. Developmentally Appropriate Expectations All rules and expectations will match the child’s developmental level. (Example: Throwing toys is not allowed because it can hurt someone.) Supportive Environment Staff will model calm and respectful behavior, offer structured routines, and provide engaging activities with meaningful choices to support positive behavior. Smooth Transitions When possible children will be given advance notice before transitions between activities to help them prepare and adjust successfully. Behavioral Screenings In support of early intervention, we require participation in ASQ-3 (developmental) and ASQ:SE (social-emotional) screenings: Screenings are required upon enrollment and if ongoing behavioral concerns arise. If areas of concern are identified, families are required to pursue appropriate services or resources. Our staff can provide referrals and assist in locating support services if needed. Group Care Environment Enrollment at Little Scholars Academy is for group care in a classroom setting, not individualized one-on-one care. While we aim to support all children to the best of our ability, our program structure does not allow for ongoing individual supervision. Same-Day Dismissal for Unsafe Behavior We understand that all children can have difficult days. However, if a child’s “hard day” behavior significantly disrupts the classroom or prevents staff from safely caring for the group, the child will be sent home for the remainder of the day. This ensures the safety and wellbeing of all children and staff in our care. Ongoing Communication with Families Parents will be informed if behavior concerns are noted. We are committed to working collaboratively to create a consistent and supportive approach between home and school. Continued Enrollment and Safety If serious behavior issues persist that pose a safety risk or cause significant disruption: A screening will be required to determine if there are any resources that may be available to help. If results indicate concerns, parents must obtain professional support and show improvement within a reasonable timeframe. If no progress is made or the behavior creates an unsafe environment, the child may not be able to remain enrolled. Should enrollment need to be terminated we will make an effort to give a notice to find other arrangements, however, depending on the severity of the behaviors exhibited this may not be possible in order to ensure the safety and wellbeing of the group as a whole. Best Interest of All Parties All decisions regarding continued enrollment will be made in the best interest of the child, the classroom, and the facility as a whole. Our goal is always to support children and families, while also protecting the wellbeing of the group. Probationary Enrollment Period All new enrollments are considered probationary for the first 60 days. LSA Child Abuse and Neglect Prevention Policy
This policy is now being distributed to all families in our care in accordance with new federal mandates that have increased the required training for childcare workers regarding the ability to recognize and report suspected child abuse and neglect. Childcare workers are in a unique position to recognize victimized children. Because of this, we are legally mandated reporters of child abuse and neglect. Annually, all staff members review the guidelines for recognizing child abuse. All staff and volunteers are mandated by law to report any suspicion of child abuse or neglect. Abuse may be physical, emotional, or sexual. Neglect is failure, refusal, or inability, for reasons other than poverty, to provide necessary care including but not limited to, hygiene, food, clothing, shelter or medical care. Staff and volunteers who report in good faith are immune from civil or criminal liability. Staff or volunteers who intentionally fail to report suspicion are subject to fines or imprisonment under the law. A report of child abuse is not an accusation. It is a request for more information by a reporter who has reasonable suspicion that abuse or neglect may be occurring. A report does not mean that our employees must determine that abuse and/or neglect has occurred. In Idaho, Child Protective Services is responsible for that determination. It is very important to us that all the children in our care are safe, and that when leaving our care they get home safely because of this the staff of Little Scholars Academy (LSA) will contact local police and/or the other custodial parent should a parent appear to the staff of LSA to be under the influence of drugs and/or alcohol. The parent’s right to immediate access does not permit the agency from denying a custodial parent access to their child even if the parent is or appears to be impaired. However, LSA staff will delay the impaired parent as long as possible, while contacting the other parent, the local police and/or Child Protective Services. Any other authorized person who attempts to pick-up a child, and appears to the staff of LSA to be under the influence of drugs and/or alcohol will be denied access to the child. The staff of LSA will contact the child’s parents, local police and/or Child Protective Services to notify them of the situation.

LSA Biting Policy
This policy aims to address biting behavior in preschool/daycare settings, acknowledging that it can occur among both toddlers and babies. It emphasizes that daycare providers are not considered negligent when a child bites another due to the spontaneous nature of such incidents. The policy focuses on prevention, intervention, communication, and support to ensure the safety and well-being of all children involved. Which will also mean that we will not disclose the name of the biter so hard feelings are not developed. These are children who have not developed a conscious understanding of how their behavior affects others yet, so we will not reveal them to the parents of the child who has been bitten. Please understand this as if it were your child. No parent wants their toddler labeled. ● Prevention Measures: ○ Provide appropriate teething toys and alternative activities to redirect biting impulses. ○ Observe children closely to identify triggers or situations that may lead to biting incidents. ○ Implement a structured routine and clear expectations for behavior to minimize frustration and conflicts ● Intervention Procedures: ○ Daycare providers will intervene promptly and calmly when a biting incident occurs. ○ Separate the involved children, ensuring the safety and comfort of all parties involved. ○ Provide comfort and support to the bitten child, addressing any necessary first aid or medical attention. ○ Comfort and redirect the biting child, emphasizing appropriate behavior and alternative ways to express themselves. ● Communication with Parents/Guardians: ○ Notify the parents/guardians of the bitten child immediately after the incident, when the skin has been broken/and or significant bruising appears quickly. Providing details of the incident and any necessary medical attention received. ○ Document the incident, including the circumstances leading to the bite, any injuries sustained, and the steps taken to address the situation. ○ Arrange a meeting or discussion with the parents/guardians of the child who bit and the bitten child to share information, address concerns, and develop a collaborative plan moving forward. ● Collaborative Strategies: ○ Work with parents/guardians to identify potential triggers or patterns of biting behavior and develop strategies to address them. ○ Provide resources and educational materials on child development, biting behavior, and positive discipline techniques to parents/guardians. ○ Foster open communication and partnership between daycare providers and parents/guardians to ensure consistency in addressing biting behavior, while still understanding that if a child bites 3 times in a day or has repeated attempts requiring one on one care to prevent biting, removing the child from the facility for the remainder of the day will be a result. ● Staff Training and Support: ○ Ensure all daycare providers receive training on understanding and managing biting behavior in toddlers and babies. ○ Provide ongoing support, guidance, and professional development opportunities related to behavior management. ○ Encourage daycare providers to share experiences, strategies, and best practices for addressing biting incidents within the daycare community Conclusion: This biting policy acknowledges that biting behavior can occur among toddlers and babies in daycare settings, emphasizing prevention, intervention, communication, and support. By implementing these policies and procedures, daycare providers can effectively manage biting incidents while ensuring the well-being and safety of all children involved. It is crucial to maintain a non-negligent approach, recognizing that biting incidents often occur spontaneously and require a collaborative effort between daycare providers and parents/guardians to address and prevent future occurrences.With this said, it is not common practice to remove a child permanently from care because of the biting. Until all measures are exhausted or there is an imminent danger to other children that is not our “jump to” procedure. We prefer to work with everyone involved and come up with the proper solution. We appreciate your patience and understanding and your kindness concerning this issue, not only to the children but to the providers as well. A lot of the time, when a child bites the teacher immediately feels like they have failed the child and the child who has been bit. This is never the case.
